14, 2005 - Landmark Health Solutions, a long-term care management
and consulting firm, signed a 5-year contract with Colonial Poplin Nursing
Home and Poplin Way Assisted Living Facilities, both currently owned
by brothers Jeffrey and Justin Philbrick. As part of the agreement Landmark
Health will manage the 50-bed nursing home and rehabilitation facility
along with the 18-bed assisted living home both located in Fremont,
NH.
“We
wanted to maintain a personalized and family-like environment,”
said Co-owner Justin Philbrick. “It was imperative that we partner
with an organization that could maintain this atmosphere and also bring
solid and reputable management experience to the table. Landmark fits
all these requirements.”
In 1983, the Philbrick's parents, Karen and Russell Philbrick, fulfilled
a dream of opening a nursing home that provides high quality, long-term
nursing care in a relaxed, home-like setting. In 2002, they complemented
the Medicare and Medicaid certified nursing home with an assisted-living
facility to strengthen the continuum of long-term care offered to the
elderly residents of surrounding communities. Both facilities emphasize
the home-like, family atmosphere along with a skilled nursing staff
and enriching activities.
