Lee Moore, R.N., C., Director, Consulting Services

With twenty-five years of nursing experience, Ms. Moore is Landmark's expert in Joint Commission accreditation, OBRA compliance and Massachusetts Department of Public Health Survey regulations. She specializes in accreditation preparation, program development, and survey readiness. Prior to joining Landmark in 1998, she was co-founder and Vice President of Partners in Practice, an independent consulting firm. While there, she opened numerous hospital-based transitional care units, handling Certificate of Need approval, managing the licensure process, and implementing all required clinical systems. She was responsible for successfully achieving Joint Commission accreditation in long-term care, dementia special care, and subacute care at twenty facilities. Ms. Moore is a graduate of St. Elizabeth's Hospital School of Nursing.

Caroli Flodstrom, R.N., M.S., Manager, Consulting Services

As Manager of Consulting Services at Landmark, Ms. Flodstrom is responsible for program development, quality systems development, survey management, and educational programming for Unit Managers, Staff Development Coordinators, and Directors of Nursing. Previously, she served as Vice President for Clinical Services at both Oakwood Living Centers and Prism Care Centers, with responsibility for clinical and operations management of several skilled nursing and subacute facilities. In addition, she was part of the research team at Hebrew Rehabilitation Center for the Aged as Manager of Field and Intervention Studies. Ms. Flodstrom holds a Bachelor of Science degree in Nursing from the University of Rhode Island and a Master of Science degree in Gerontological Nursing and Nursing Administration from the University of Massachusetts - Boston.

Jennifer A. Butt, R.N., RAC-CT, C-NE Clinical Reimbursement Consultant

Ms. Butt is a Senior Clinical Reimbursement Associate responsible for the training, oversight, and reimbursement education of MDS/PPS Coordinators as well as case mix processes for Medicaid reimbursement in the states of Massachusetts and New Hampshire primarily. Ms. Butt is certified by the American Association of Nurse Assessment Coordinators (AANAC) and has passed the advanced certification test to become an AANAC Master Teacher. Previously she has held various positions in long term care nurse leadership including Director of Nursing. She is a Certified Nurse Executive from the American Association of Nurse Executives. Ms. Butt obtained her nursing degree from Central Carolina Technical College in Sumter, South Carolina.

Tracy J. Williams-DiStefano, O.T.R. /L, Manager of Rehabilitation Services, LandView Therapy Services

An expert in PPS management, clinical supervision, staffing, and program development, Ms. Williams-DiStefano oversees therapy services at several nursing facilities and supports the Rehabilitation Directors at each nursing home. She has worked for J. Rougeau Therapy Services for seven years and was Rehabilitation Director at Harborside Healthcare in South County, Rhode Island. She holds a Bachelor of Science degree in Occupational Therapy from Worcester State College.
